Turnaround Tuesday is a community-driven jobs movement, connecting local residents to a full spectrum of workforce and leadership development while simultaneously identifying and cultivating employers in the local footprint. Since 2014, Turnaround Tuesday has connected over 600 people back into the workforce, many returning from prison and turning their lives around through training, new employment, and reengagement in their communities.
The case manager will join a diverse team of workforce staff and BUILD leaders. The case manager will assess, plan, implement, and evaluate health and human services needs of Turnaround Tuesday participants as they prepare to reenter the workforce. They will be directly involved in the lives of the participants and will be responsible for identifying barriers to employment and creating and maintaining community partnerships that provide supportive resources.The case manager must be highly organized, compassionate, nonjudgmental, and eager to support adults rebuilding their lives.
At least 2 years of experience in case management preferred.
Duties and Responsibilities
- Provides individual assessments to participants to identify immediate needs, and develops a strategy with participants to address barriers to work.
- Addresses the well-being of clients, including people with health issues, addictions, disabilities or who have experienced trauma.
- Provides, communicates, and documents a clear process with participants to connect to support and resources with timely follow up.
- Listens and provides connections for participants to workforce support team.
- Develops strong working relationships with workforce team and communicates weekly on participants’ progress.
- Accurately records case information, completes all required forms, and produces weekly statistical reports.
- Researches relevant resources such as housing, substance abuse treatment & mental health services for participants.
- Creates strong working relationships with support networks, and community resources.
- Organizes and advocates for the development of programs and services that are currently inadequate to meet the needs of participants.
- Identifies opportunities for volunteers and interns; provides training, supervision, and support.
Case Manager Requirements and Qualifications
- Experience in case management, including as a mental health care manager, nurse, social worker or related job.
- Previous experience with psychological aspects of care.
- Empathetic and supportive with mentorship and leadership skills.
- Problem solving skills and ability to multi-task.
- Excellent interpersonal skills with diverse populations (i.e. program participants, colleagues, workforce partners, employers, institutional & community leaders).
- Excellent time management skills; organized and able to prioritize.
- Motivated to find and develop resources when existing support services are inadequate.
- Comfortable in a fast-paced environment with multiple cases.
- Able to organize and manage large amounts of files, schedules, dates, and information.
- Self-directed and able to work without supervision.
- Bachelor’s degree in social work, psychology, sociology, healthcare or medical and related fields; Master’s degree in social work preferred; supervised practicum or internship preferred
- Licensed or certified by state board according to regulatory, LCSW, LCPC, LCADC, LGPC, LGSW, LGADC, and will consider candidates with CAC-AD.
- Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel)
